Optometrist of the Year
Optometrist of the Year
The Pennsylvania Optometric Association “Optometrist of the Year” Award recognizes a member of the POA who, through years of dedicated service to optometry and the public, merits recognition from the optometrists of the state of Pennsylvania.
Nominations will be accepted from the POA’s local societies and committees. Nominees will be evaluated according to a point system allocated to three principal areas. In the sections below, please describe key accomplishments, special initiatives, volunteer work, etcetera that can be attributed to the nominee in regards to the areas specified.
Only one nominee per local society will be accepted. Nominations will not be accepted if the required fields are incomplete.
Plaque Order (Optional)
To order a Local Society Optometrist of the Year Award plaque from the POA, click here and follow the instructions. The order form must be sent at least four weeks prior to when the plaque is needed.
