William Van Essen Award
The Pennsylvania Optometric Association “William Van Essen Award” honors individuals or organizations outside the optometric profession who have significantly served the visual welfare of the public.
Nominations will be accepted from the POA’s local societies. Nominees will be considered based on service to the visual welfare of the public as presented in the application. This award is not appropriate for community figures whose contributions have been in areas other than visual welfare or for legislators.
Only one nominee per local society will be accepted. Nominations will not be accepted if the required fields are incomplete.
Plaque Order (Optional)
To order a Local Society William Van Essen Award plaque from the POA, click here and follow the instructions. Please note that the Local Society’s VCI account will be charged for all costs associated with the plaque. The order form must be sent at least four weeks prior to when the plaque is needed.
